Tree of Life

Important Forms

The Tree of Life, located in the St. Rose Spirituality Center, is a special memorial for our unborn and children who have passed. The vibrant Tree of Life painting depicts St. Rose Philippine Duchesne and St. Philomena keeping careful and loving watch over our loved ones. The Tree is adorned with circular memorial ornaments that are graciously donated by our Knights of Columbus.

If you would like to remember a child with a complimentary custom engraved memorial, please download and fill out the Tree of Life Memorial Order form and bring it in person to Trish Boone at the Parish Office, or it can be faxed to 623-742-7031. The Parish Office is open Monday through Friday 8:30am to 4:30pm at 2825 West Rose Canyon Circle in Anthem, Arizona.

When the engraved memorial ornament is completed, you will receive a call and/or email requesting whether you would like to pick it up or have it mailed to you. At your convenience, you may hang it on the Tree of Life or ask that it be placed on the Tree for you.
